• Asthma Daytime Symptom Diary V1.0 (ADSD V1.0)

• Asthma Nighttime Symptom Diary V1.0 (ANSD V1.0)

• Disability Rating Scale (DRS)

• Eastern Cooperative Oncology Group Performance Status (ECOG)

• Karnofsky Performance Status Scale (KPS Scale)
• Kurtzke Functional Systems Scores (KFSS)
• Quality of Life in Inflammatory Bowel Disease Questionnaire (IBDQ)
